Xprehab Program

This technology enabled 12-week prehabilitaiton program endorsed by surgeons allows patients to work toward managing and stabilising symptoms before their joint replacement surgery. The Xprehab program aims to optimise the episode of care through remote monitoring and data analytics.

What is the Xprehab Program?

The 360 Xprehab Program (extended prehabilitation program) is a 12-week multidisciplinary program for patients with hip and knee joint osteoarthritis who have surgery planned for more than 3 months into the future. This surgeon endorsed program gives patients access to programs designed by a team of Patient Partner Physiotherapists and dietitians to assist with stabilising and managing patient symptoms whilst awaiting and preparing for their upcoming surgery.

Based on 360 Med Care’s multidisciplinary Osteoarthritis Management Program which began trials in 2018 for individuals with hip and knee osteoarthritis, the Xprehab Program provides evidence-based treatment options aimed at reducing pain, improving function and quality of life, and ultimately delaying surgery.  The Xprehab Program works in conjunction with the existing Patient Partner Program and extends episode of care management options for patients in the early stage of their joint replacement journey.

What’s included in the Program?

Throughout the 12-week program, patients will be guided through a structured and individualised exercise management program in addition to having access to exercise, health living, and dietetics modules based on their current limitations and future goals. The surgeon remains actively engaged, and when necessary, a patient’s care can be easily escalated back to the surgeon to allow for further consultation. Outcome measures will be completed at various stages throughout the program and will include but not be limited to:

  • Patient Reported Outcome Measures (PROMs)
  • Joint range of motion
  • Strength
  • Balance
  • Function

The 360 Xprehab Program is delivered entirely via telemedicine, increasing comfort and convenience for patients. This remote delivery allows the  patient and their Patient Partner Physiotherapist to remain in close contact throughout the duration of the program and collaborate to meet their goals.

Depending on your health cover, the 360 Xprehab Program might not cost anything. Contact your current provider to confirm whether you are covered.
Yes. The Patient Partner who will be the primary care coordinator for the program is a highly trained physiotherapist that has advanced knowledge and understanding of osteoarthritis in the hip and knee.
Not necessarily. The 360 Xprehab Program is designed to assist with stabilising and managing current pain and symptoms and may benefit your level of function. The degree of improvement will be dependent on your current symptoms and degree of pathology.
Nowhere. The 360 Xprehab Program is delivered entirely remotely meaning patients can receive care in the comfort of their home. Through the use of advanced technologies, the Patient Partner is able to assess and treat symptoms without needing to see the patient in person.
Yes. One of the rationales behind the 360 Xprehab Program is to not only manage current symptoms but also improve levels of strength, flexibility and function so that should a joint replacement be required, patients are in a positive position to have an optimal outcome.
